Community Oncology & Palliative Care

The Community Multidisciplinary Specialist Oncology and Palliative Care Team work together, supporting and complimenting the core community services, 24hr nursing and the district nursing teams.

The team provides service to patients and families who are experiencing and managing complex needs as a result of cancer and other life-limiting conditions, helping to prevent unnecessary or avoidable admissions to hospital.

Our overall aim is to maintain and improve quality of life by:

  • Provide timely response to referrals from health and social care professionals and members of the general public
  • Offering specialist guidance on symptom control
  • Offering advice on accessing services
  • Help plan and co-ordinate specialist care
  • Providing expert clinical support and education to other health and social care professionals.
  • Where possible fast track requests for equipment
  • Work in partnership with all other organisations involved in the care of patients with cancer and life limiting illness.



How to Access the Service

Referral to this service can only be made by hospitals, GPs and other healthcare professionals.
