Transforming Your Care

In June 2011, the Minister for Health, Social Services and Public Safety, Edwin Poots, MLA, announced that a Review of the Provision of Health and Social Care(HSC) Services in Northern Ireland would be undertaken. The Review was to provide a strategic assessment across all aspects of health and social care services, examining the present quality and accessibility of services, and the extent to which the needs of patients,clients, carers and communities are being met.

Belfast Trust (and the other 4 Trust's in Northern Ireland) have now produced draft population plans that identify which services and facilities are needed in each area to address the needs of the local population and set out how this can be delivered.  these plans are now available on the DHSSPS website to facilitate a period of quality assurance of the documents and further engagement with clinical leaders before the formal consultation process begins in September. This means the draft Plans may be subject to change, but will be finalised after the autumn public consultation process.

Integrated Care Partnerships

Integrated Care Partnerships (ICPs) are a key element of Transforming Your Care, and will be a new way of working for the health service in Northern Ireland to transform how care is delivered.
