Have you raised a complaint regarding Health and Social Care since 1 April 2009?
The Health and Social Care Board is undertaking an evaluation of the complaints procedure which was implemented on April 1 2009 by Health and Social Care organisations for managing complaints.
Would you be willing to complete a questionnaire to provide us with feedback on your experience of the complaints process? Your participation in completing this questionnaire would enable us to learn from the experience of Health and Social Care organisation service users. Please note that participation in this questionnaire is completely voluntary.
