Community Nursing ServicesPrimary Care Teams comprising general practitioners, nurses, occupational therapists, dental practitioners, physiotherapists, speech and language therapists, podiatrists, dietitians and social workers provide primary care services to the entire population of North and West Belfast. Our vision for the future is underpinned by the need to further strengthen the way in which our primary care teams are organised, to maximise the delivery of fully integrated health and social care services: • to work closely with the Local Health and Social Care Group to identify and target needs within local communities; • to contribute to local strategies for health and wellbeing using the experience of patients and clients; • to provide services in the community rather than in institutions; • to ensure equitable access to the full range of primary care services by increasing efficiency and improving accessibility; and • to build and extend the range of specialist clinical services accessible to the local population |
