DISCHARGE FROM MUCKAMORE ABBEY HOSPITAL When you are ready to leave Muckamore Abbey Hospital the staff team who looked after you will help to make your move to the community as easy as possible. You and you relative or carer will be involved in decisions about your move from the Hospital and where you will live in the community. Staff in the community will also be fully involved in planning for your discharge. If you need support after you leave hospital then this will have been agreed with your local community services. Where you are moving to a new home in the community this will often take place over many weeks so that you have a good chance to try it out and to settle in to your new environment.
A letter will be sent to your doctor to explain the treatment you received in hospital and to advise on any ongoing treatment you may need. If you are on medication you will be given a supply until you see your own General Practitioner. Finally do not forget to take all your personal belongings with you. |
