Services for Older People
Our focus here will continue to be on the development of community services which enable elderly people to remain in their own homes, living as independently as possible. These services will include home help, intensive domiciliary care, day care and respite services with the support of district nursing, occupational therapy, physiotherapy, community nutrition and dietetics and podiatry services.
Residential, nursing and intensive domiciliary care for elderly people will remain a priority for the Trust. These services will be provided by a range of statutory, voluntary and private organisations and will be targeted at the most frail and dependent elderly people in our community.
The Trust will continue to develop schemes to facilitate the early discharge of elderly people from hospital and will collaborate with General Practitioners and other healthcare professionals in providing a level of care which will prevent unnecessary admission to hospital or long-term care. |
