Community Stroke Rehabilitation Team

	Service Provided The ethos of the Community Stroke Rehabilitation Scheme is to offer individuals the choice to have their acute rehabilitation, following a stroke, at home rather than in a hospital based setting. The team includes a Speech & Language Therapist, Physiotherapist, Occupational Therapist, Social Worker and Rehabilitation Assistants.
Accessing the Service: Referral to this service can only be made through the consultants at the Royal Victoria, Belfast City and Ulster Hospitals.
