Community Mental Health Team

	Service Provided The team consists of Social Workers, Community Psychiatric Nurses, Support Workers and an Occupational Therapist. They provide assessment, education, advice and support to individuals over the age of 65 who are experiencing a mental illness and individuals of any age who are experiencing or have concerns about Early Stage Dementia.

Carers Services Carers Assessments A Carers Needs Assessment will look at the care or support you provide as a carer and how it affects your life. It will explore what support you have and what support you may need to help you if you want to continue caring for your relative or friend. It is an assessment for you as a carer, not the person you care for. The course lasts for 7 weeks and is available 3 times a year. They will provide education and support in relation to the condition of the person they are caring for and develop strategies to enable them to care effectively whilst also looking after their own needs.
