Community Mental Health Teams

Community Mental Health Teams are usually made up of Community Psychiatric Nurses, Psychiatrists, Social Workers and in some cases support workers.

They provide community-based support to individuals to enable them to cope with their experience of mental illness. They do this through assessment, education, advice and therapeutic support.

The team can also access a range of other services such as accomodation and community support.

How to access the Service

Community Mental Health Team for Adults aged 18-65 years
Referrals to this service can only be made by GPs or other healthcare professionals through One Point of Referral. 

Community Mental Health Team for Older People
This service operates an open referral system for individuals over the age of 65 who are experiencing a mental illness and individuals of any age who are experiencing or have concerns about dementia.

Under 18s
For those under 18 years of age, support and help to cope with the experience of mental illness is available from the Community Adolescent Mental Health Service.
