Community Respiratory Nursing Team

The Community Respiratory nursing team provides care for adult respiratory patients within north and west Belfast. The team is based in Twin spires.

COPD is the primary condition treated by the team other conditions include Pulmonary Fibrosis, Chronic Asthma and Bronchiectisis.

The service aims to provide evidence based care for respiratory patients in their own homes and at clinics. Menu of services provided include:

• Respiratory Enhanced Discharge
• Prevention of hospital admission
• Chronic disease management
• Case management of complex cases
• Palliative care.

How to Access the Service

Patients must live in north or west Belfast, and have a confirmed diagnosis of respiratory disease. The reason for referral should be related to their respiratory condition.

Referral to this service can only be made by hospitals, GPs and other healthcare professionals.
