Condition Management Programme
The Condition Management Programme (CMP) is a 12 week voluntary programme delivered by health care professionals, which aims at helping people better understand and manage their health symptoms and return to a healthy lifestyle.
The programme supports participants as they work to improve their quality of life and realise their potential so they can move closer to employment now or in the future.
This programme is part of the Department for Employment and Learning’s Pathways to Work initiative, and is delivered by the Trust in partnership with the Disablement Advisory Service. Learn more about Pathways to Work
What does CMP offer?
The CMP team is made up of physiotherapists, occupational therapists, mental health and general nurses, and can access other health care services as required. We provide a range of interventions including:

What sort of health conditions can be tackled?
Health care professionals will provide support and advice to help clients with a range of conditions including:
- heart and circulatory conditions
- respiratory disorders
- mental health problems such as depression, stress
- drug and/or alcohol use
- musculo-skeletal conditions such as arthritic complaints, back and neck problems,
- and other conditions including chronic fatigue, pain, diabetes and epilepsy.

Accessing this Service
Anyone on Incapacity Benefit or in receipt of Employment Support Allowance can take part in the programme.
If you would like to participant contact an Employment Advisor at your local Jobs and Benefits Office. The advisor can then refer you to the CMP.
If you agree to participate in CMP, an individual action plan will be agreed between you and the healthcare professional.
Your GP will then be advised by letter that you have volunteered to attend the Programme. They will also be told when you complete the programme.

CMP will not be suitable for everyone, but there are lots of other options open to you through the Pathways to Work programme. Your Employment Advisers will provide information on all the other choices, or you can find out more from NIDirect.
