Homecare/Home Help Services

Homecare/Home Help Service is provided in the form of personal care and/or a domestic service to individually who because of their frailty or disability could not be maintained at home.  Eligibility for the service is determined following an assessment of needs and the level of service provided will depend on the presenting problems and what other sources of support are available.

A domestic service will be provided if practical tasks such as fire-lighting or assistance with meals is required.  A personal care service will be provided if help is required with tasks such as getting in and out of bed, washing and dressing.

Intensive Homecare

The Intensive Homecare provides a flexible, responsive service to enable older people to remain in their own homes for as long as possible.

The service is provided to people aged 65 and over who have a range of diverse health and social care needs and require the assistance of two staff at all times.

An individualised package of care over a 24-hour period is provided based on the needs of the older person, to enable them to remain in the community rather than going into residential or hospital care.  The service also offers support to their carers.

The Intensive Domiciliary Support Scheme provides assistance with all aspects of daily living including:

  • Assistance with personal care tasks
  • Assistance with all transfers
  • Assistance with medication
  • Assistance with meal provision
  • Household tasks
  • Shopping
  • Carer Relief

How to Access the Service:

Referrals to this service can only be made by a health care professional.

They will contact a care manager who will arrange for a multi disciplinary assessment to be carried out.  This will include assessments from your GP, a social worker, nurse, physiotherapist and occupational therapist . Following this, agreement will be reached as to the level of services provided, considering other services which may be available.
