Homeless team

This Service provides public health care services for single homeless people living in temporary hostel accommodation, and rough sleeping in Belfast.

The aim of the service is to improve the health and health care experience of this group of people.

“Door Step” delivery brings a range of services to the homeless in places that they feel comfortable, safe and willing to engage.  These include:

  • One to One health Clinics
  • Health Assessments
  • Health Screening
  • Specialist Clinic Sessions
  • Health Education/Promotion programmes tailored to match assessed health needs
  • Coordination of Allied Health Professionals e.g. Podiatry Dental  Ophthalmology services
  • Referral Pathways to other Health and Social Care Services
  • Advice and support for homeless clients, hostel staff and others involved in homeless issues.
  • Training for Hostel Staff in health issues
  • Awareness raising of homeless issues at local and  strategic levels
  • Advocacy used as a means to empower the homeless in decision making and healthier life choices

How to access this service

This service is freely available to those with health and social care needs who are without a home, living in temporary hostel accommodation, day care and night crash facilities or who are rough sleeping in Belfast.

Referrals can be taken from any member of the public or from the following services:

  • Call Management
  • Social Services
  • Hostel Staff
  • Multiagency /disciplines involved in homeless services
  • GPs
  • Prison Services
  • Hospital Professionals
