Integrated Care Teams

This section provides information about integrated care teams that provide a range of health and social care services including community nursing for people living in south and east Belfast.  If you live in north or west Belfast, find out about your community nursing services here.

Many people need both health and social care support. In Integrated Care Teams a range of health and social care staff work together  to offer patients a better quality service, with easier access to the services required.  There are five Integrated Care Teams based in south and east Belfast.  Treatment and Services are provided through district nursing, Occupational Therapy, Care Management and Social Care staff.  Nursing services are available 24 hours a day, every day and include a rapid response service for GPs and Emergency Departments.

  • District Nursing
    District Nurses in the Integrated Teams provide nursing care, support and advice for individuals, carers, families and the wider population. They are based in five teams and aligned to a GP Practice in a defined area and provide 24 hour care to housebound patients
  • Occupational Therapy
    Occupational Therapists in the Integrated Teams provide a service to older people and those with a physical disability, incorporating assessment and treatment in client’s own home, residential or nursing home to enable people to live as independently as possible
  • Care Management
    Care Managers in the Integrated Care Teams provide an assessment for those with complex needs, requiring a high level of support, either at home or in a residential or nursing home.
  • Social Work
    Social Work staff in the Integrated Teams provide an assessment for those with complex social needs and provide social services to support patients with such need.
  • Clinical Support Team (Nursing)
    This Nursing Team completes Clinical Investigations, Post-operative wound checks and follow up dressings, suture/clip removal, routine injections and equipment and continence monitoring.

 


 

How to Access the Service:

You can refer yourself or be referred by a family member, GP. or by other health professional. 

All referrals are handled centrally by a Call Management Centre 90 565565 which is available 24 hours a day
